Child ADHD Screening Questionnaire

The following questions are based on well-established criteria used by clinicians to screen for Attention-Deficit/Hyperactivity Disorder (ADHD). Please answer each question based on how your child has behaved over the past 6 months, compared with other children of the same age. This screener takes approximately 2–3 minutes to complete and is designed to help identify whether a full ADHD assessment may be helpful. This screener does not provide a diagnosis.
